Notes of exception: 

A child who commenced vaccinations (start-over) after 7 years of age would need only 1 Tdap followed by 2 TD doses with the 2nd TD given 6 months after the 1st.

DTP is acceptable for DTaP.

* 3 Polio doses are accepted only if the last dose is given at or after 4 years of age and 6 months after the 2nd dose was given.

** 4 DTaP doses are accepted only if the last dose is given after 4 years of age and 6 months after the 3rd dose was given.

If a student requires daily or emergency medication during the school day, a District Medication Authorization Form must be completed and signed by the health care provider each school year. The parent must bring all medications to the school with the completed medication authorization form. Medications must be in the original container and labels must match the health care provider’s order. 

Please inform the school nurse if your child requires any special nursing procedures such as diabetes care, tube feeding, catheterization, etc. Please have the appropriate form completed by your child’s healthcare provider, and bring it to school with any supplies that are needed for the procedure. 

The school district coordinates with the Florida Department of Health-Collier County to provide state-mandated health screenings. If you do not wish your child to participate in one or more health screenings, you must complete a Parent Opt-Out form and provide to the school in writing prior to the screening date. 

Other partner organizations providing health-related services to students include:

• Bascom Palmer Eye Institute, University of Miami provides vision screenings for students in grades K-12 in Title 1 schools.  Free eye examinations and two (2) pairs of glasses, if needed, are provided for students that are referred based on the screening.
• University of Florida, College of Dentistry provides dental screening of all students in grade 3.  Dental sealants for first and second grade students at select schools.
• Health Care Network, Ronald McDonald Care Mobile provides dental sealants for second grade students at select schools
   

Florida Administrative Code & Florida Administrative Register 64F-6.003 Screening

(1) Vision screening shall be provided, at a minimum, to students in grades kindergarten, 1, 3 and 6 and students entering Florida schools for the first time in grades kindergarten through 5.

(2) Hearing screening shall be provided, at a minimum, to students in grades kindergarten, 1 and 6; to students entering Florida schools for the first time in grades kindergarten through 5; and optionally to students in grade 3.

(3) Growth and development screening shall be provided, at a minimum, to students in grades 1, 3 and 6 and optionally to students in grade 9.

(4) Scoliosis screening shall be provided, at a minimum, to students in grade 6.
